Returning Rostered Musicians
RETURNING AS A ROSTERED MUSICIAN? Welcome Back!
​
BEFORE the FIRST REHEARSAL of every season, you are required to read the current WVS Employee Handbook. The Handbook changes annually. Therefore, each season, you are required to sign and return an updated:
​
ACKNOWLEDGEMENT OF EMPLOYEE HANDBOOK RECEIPT which is included in the Employee Handbook. You will find the Acknowledgement form on Page 22 of the 2023-2024 Employee Handbook. THEN, after you've read the Handbook, print the form, sign it and return as instructed. The link to the Acknowledgement form to print and complete is below.

IF YOU HAVE MOVED, CHANGED BANKS OR WANT TO CHANGE WITHHOLDING SINCE YOUR LAST PAYROLL, please complete the following packet of forms including a W4, A4 and Direct Deposit form. The link to those forms is below.

Returning Sub Musicians
RETURNING AS A SUB MUSICIAN? Welcome Back! (Returning Sub means you played with us in 2021/2022 or 2022/2023).

BEFORE the FIRST REHEARSAL OR CONCERT THAT YOU WILL SUB IN, you are required to read the current WVS Employee Handbook. The Handbook changes annually. Therefore, each season, you are required to sign and return an updated:

New Musicians
NEW MUSICIAN? Welcome! (If this is your first time playing with the WVS as a Rostered Musician or Sub, here is everything you need to know (and the required forms to complete)!
​
BEFORE your appearance at the FIRST REHEARSAL OR CONCERT THAT YOU ARE PERFORMING IN, you are required to read the current WVS Employee Handbook. You are also required to complete, sign and return the following documents, which are included in the New Musician Packet. You’ll find the link below these instructions.
​
1. ACKNOWLEDGEMENT OF EMPLOYEE HANDBOOK RECEIPT FORM. It is on Page 22 of the current WVS Employee Handbook.
​
2. The West Valley Symphony participates in the Department of Homeland Security, U.S. Citizenship and Immigration Services E-Verify program. An I-9 Employment Eligibility Verification form must be completed, and the proofs listed on page 2 must be provided.

I-9, Section 1: Complete carefully and return required paperwork and proofs:

If you are in the country as shown in Item (3), A lawful permanent resident, please enter a USCIS or A-number and include proofs.

If you are in the country as shown in Item (4), A noncitizen authorized to work, complete the expiration date AND these items: USCIS A-Number, OR Form I-94, OR Foreign Passport Number and include proofs.

3. The WVS pays only by Direct Deposit, so please complete the Direct Deposit form, Federal W4 and State of Arizona A4 withholding documents.
